To intensify the city’s epidemic prevention and control efforts and properly carry out the work on changing the color of Jiangsu Health Code in due course, the followings are hereby notified: 

I. Scope of Application

1. For “yellow” health code holders who have no travel history to medium-risk or high-risk areas or Nanjing Lukou International Airport, they can turn to local residing communities to apply for a change of the color of their health code if they could make a commitment on their health status and travel history, and show two negative nucleic acid test results (the two tests should be no less than 24 hours apart) as required. 

2. For those with a travel history to medium-risk or high-risk areas, if they have finished 14-day medical observation at designated places plus 7-day health monitoring at home and have tested negative in three nucleic acid tests, they can turn to local residing communities to apply for a change of the color of their health code. 

3. For inbound travelers, if they have finished 14-day medical observation at designated places plus 14-day health monitoring at home and have tested negative in three nucleic acid tests, they can turn to local residing communities to apply for a change of the color of their health code. 

4.For those with a travel history to medium-risk or high-risk areas in Nanjing or Yangzhou, if they have finished 14-day medical observation at designated places plus 14-day health monitoring at home and have tested negative in three nucleic acid tests, they can turn to local residing communities to apply for a change of the color of their health code. 

II. Procedures

1.Applicants should bring relevant supporting documents and turn to local residing communities;

2. Community workers review their application as per the aforementioned requirements and forward to health code operators of local sub-district after approval;  

3. Health code operators report to higher authorities via the health code system;

4. Local district (county-level city) authorities review the application and change the color of health code.

IV. Attention

「蘇康コード」カラーの規範的変換に関する重要業務注意 

蘇州市の新型コロナ感染の予防·抑制対策を強化し、「蘇康コード」のカラー変換業務をタイムリーに展開するため、以下の業務関連事項を注意する。

一、申請範囲

1.中·高リスク地域での旅行歴及び南京禄口空港での通過歴がないイエローコードの所持者、本人が承諾し検証を受けた上、要求に応じて2回のPCR検査証明書（24時間以上の間隔）を提示できる場合、コミュニティでコードカラーの変換を申請できる。

2.中·高リスク地域での旅行居住歴がある人、14日間の集中隔離医学的観察及び7日間の在宅健康モニタリングを完了し、且つ3回のPCR検査を受け、結果がすべて陰性であった場合は、コミュニティでコードカラーの変換を申請できる。

3.入国者の場合、14日間の集中隔離医学的観察と14日間の在宅健康モニタリングを完了し、且つ3回のPCR検査を受け、結果がすべて陰性であった場合、コミュニティでコードカラーの変換を申請できる。

4.南京市と揚州市の中·高リスク地域での旅行居住歴がある人、14日間の集中隔離医学的観察と14日間の在宅健康モニタリングを完了し、且つ3回のPCR検査を受け、結果がすべて陰性である場合、コミュニティでコードカラーの変換を申請できる。

二、変換の流れ

1.コード所持者は相応の証明書類を持ち、居住地が所属するコミュニティに行って申請する；

2.コミュニティのスタッフが上記のコード変換規定に基づいて審査を行い、審査通過のケースを街道の蘇康コード操作員に報告する；

3.街道の蘇康コード操作員がシステムを通じて区（県）に報告する；

4.区（県）が最終的に審査して、コードの変換を完成する。

四、注意事項

団体のカラー変換を申請する場合、各地域より2021年7月28日に蘇州市感染予防・抑制コントロール指揮部社会とコミュニティコントロール組が公布した「蘇康コードカラーの団体変換をめぐる業務メカニズムの明示に関する通達」（市社会とコミュニティ組〔2021〕第5号）を厳格に実施する。
